Hospice Services
Redwood Hospice recognizes that selecting hospice is the beginning of a care partnership – and that our physicians need to be present to provide that care.
Our Redwood Hospice team provides:
- Care at home – wherever the patient resides
- On-call hospice nurse support – 24/7
- Medical equipment – to use while in hospice
- All hospice-eligible medications
- Pain/symptom management – ensuring comfort and peace
- Social and Spiritual care for patient and family to ensure emotional safety and comfort
- Respite care – so family caretakers can have a rest
- Grief and Bereavement support for the surviving caregiver and family for up to 13 months after the death of a patient.
Our partnership with you includes:
- An assessment of the physical, medical, psychological, social and spiritual needs of the patient and the patient’s family
- A plan which includes coordinated care emphasizing supportive services, like home care, pain control, and some inpatient services
- Palliative medical treatments to relieve pain and other symptoms
- Bereavement services to support the family and caregivers with social and emotional needs.
Every Redwood Hospice patient and family will have a hospice physician to visit them wherever they call home when they start hospice care. This physician will follow and manage their care with the team of other clinicians throughout the individual’s hospice experience.
