    We will ask you a few questions, assess your personal needs, and customize a plan that fits your specific needs.

    Follow-up

    Your Personal Care Manager will oversee your care and make sure you have everything you need.

Our team consists of the patient's Primary Care Physician, the hospice visiting physician, medical director, RN case managers, licensed vocational nurses, hospice home health aides, social workers, chaplains, and trained volunteers.

My veteran father came home on hospice. The original hospice team placed by the Veterans Affairs Hospital was utterly disorganized and chaotic.
Within minutes of talking to Sarah J. at Redwood Hospice listening to their overall approach and services given to their patients as well as their structure of services, I knew my fathers care as well as my families care would be priority and listened to.
Within hours of contacting Redwood Hospice we had Dr. Joshi, Nurse Janet and Ms. Sarah J come to our home and calmly go over services, schedules, game plan and execution in such an organized yet gently and understandable way for my family to process and take in.
My entire family (mother and 3 brothers) felt at ease knowing dads new team would offer him the compassionate care he deserves. It’s been two days and we’ve had constant communication, necessary visits and schedules for care in place for dad. Thank you Sarah J!
